RSLT CrossFit Liability Waiver and Release Form

In consideration for being allowed to participate in the activities and programs offered by RSLT CrossFit, I, the undersigned, acknowledge, appreciate, and agree that:

1. Assumption of Risk:
I understand and acknowledge that the activities and exercises offered at RSLT CrossFit include, but are not limited to, weightlifting, cardiovascular exercises, bodyweight exercises, high-intensity interval training, and other fitness activities. I am aware that these activities involve inherent risks, including the risk of serious bodily injury or death.

2. Physical Condition:
I hereby represent that I am in good health and have no physical condition that would prevent my participation in the activities and programs offered at RSLT CrossFit. I acknowledge that I have been advised to consult with a physician prior to and regarding my participation in any exercise program.

3. Release and Waiver:
I, on behalf of myself, my heirs, executors, administrators, successors, and assigns, hereby release, waive, discharge, and covenant not to sue RSLT CrossFit, its owners, officers, employees, and agents from liability for any and all claims, demands, losses, or damages on account of injury, including, but not limited to, personal injury, property damage, or death, caused or alleged to be caused, in whole or in part, by the negligence or other conduct of RSLT CrossFit or its employees or agents, or by any other person using the facilities of RSLT CrossFit.

4. Indemnification:
I agree to indemnify and hold harmless RSLT CrossFit from any loss, liability, damage, or cost they may incur due to my participation in the activities and programs offered at RSLT CrossFit.

5. Photographic Release:
I grant RSLT CrossFit the right to use photographs and/or videos of me taken during my participation in RSLT CrossFit activities for promotional purposes, including but not limited to, social media, website, and other marketing materials.

6. Cancellation

Please note that a 1-month notice is required for membership cancellations. Your membership will stay active for 30 days following your cancellation request, allowing you to continue using all benefits and services during this period. Thank you for your cooperation.

I have read this waiver and release form in its entirety and fully understand its terms. I understand that I am giving up substantial rights, including my right to sue. I acknowledge that I am signing the agreement freely and voluntarily, and intend by my signature to be a complete and unconditional release of all liability to the greatest extent allowed by law.
