Physical Activity Readiness Questionnaire (PARQ)
This PARQ is designed to help you help yourself. Many health benefits are associated with regular exercise and this questionnaire is a checklist to complete before you undertake new or additional physical activities. For most people, physical activity should not pose any problems or hazards. This PARQ has been designed to identify anyone who should seek medical advice concerning the type of activity most suitable for them.
If you are aged between 15 and 69, the PARQ will tell you if you should check with your doctor before you significantly change your physical activity patterns. If you are 69+ years of age and aren’t used to being physically active, check with your doctor before undertaking additional or new forms of exercise.
Please read each of the following questions carefully and answer honestly by indicating YES or NO
If you answer YES to one or more questions:
You should consult with your doctor to clarify that it is safe for you to become physically active at this time and in your current state of health.
If you answer NO to ALL of the questions:
It is reasonably safe for you to participate in physical activity, gradually building up from your current ability level.
Note: This PARQ becomes invalid should your condition change
Do you consent to us taking your temperature?
Do you have any of the following symptoms: new or persistent cough, high temperature, loss of taste or smell? Do you live with someone who has any of these symptoms?
Are you currently awaiting a test for COVID-19?
Are you living with anyone who is self isolating?
Have you been contacted by the government through the ‘Track & Trace’ Scheme in the last month?
Have you had or been tested positive for COVID-19 in the past 4 weeks?
Have you experienced chest pain when you exercise?
Do you have any heart conditions?
Are you or have you been pregnant in the last 6 months?
Do you lose balance because of dizziness or do you ever lose consciousness?
Do you have a bone or joint problem (for example arthritis or joint replacement) that could be made worse by a change in your physical activity?
Is your doctor currently prescribing medication for a blood pressure or heart condition?
Are you currently taking any form of medication?
Do you currently have a disablitiy or communicable disease?
Do you know of any other reason why you should not take part in physical activity that may endanger your physical and mental health/ wellbeing?
Having answered YES to one of the questions above, I have now sought medical advice and my GP has agreed that I may exercise.
