This Informed Consent and Assumption of Risk and Release of Liability is entered by and between Ignite Fitness LLC and you, the “Client”.

Client certifies that the Client:
-is of adequate physical condition to participate in physical exercise.
-assumes the risk of physical injury, whether minor, severe, or otherwise.
-will disclose to the Trainer whenever suggested activities cause distress beyond Client’s threshold.
-will not hold the Company or its Trainer(s) liable for any physical injury, whether minor, severe, or otherwise that result from Training Sessions.
-assumes all responsibility for Client’s participation in the Training Sessions.

The business shall not be liable for any loss or theft of my personal property. I also agree that the business shall not be responsible for all such personal or property injuries, damages, loss, or theft, even in the event of negligence, fault or failure to use due care on the part of the business, and whether such negligence, fault or failure to use due care is present at the signing of this agreement or takes place in the future.

I consent to the business taking photos or video image of me while in classes or otherwise in the public premises of the business and hereby irrevocably grant the right to use my image or likeness in such images or photographs in any advertising, promotion, marketing materials including any and all social media, print media, or on the website.
