CrossFit Deltafox - DeltaFox PAR-Q and Waiver

By visiting/taking part at CrossFit DeltaFox, you are agreeing to participate in physical fitness activities which are of a nature that is strenuous and may push you to the limits of your physical abilities. These activities are not without varying degrees of risk which could include but are not limited to:

- Injury to the musculoskeletal and/or cardio respiratory system which can result in serious injury or death.
- Injury or death due to negligence on the part of yourself, your training partner, or other people around you.
- Injury or death due to improper use or failure of equipment.
- Injury or death due to a medical condition, whether known or unknown to you.

By visiting/taking part at CrossFit DeltaFox, you are agreeing that:

- it is your sole responsibility to obtain an examination by a doctor prior to involvement in physical activity or an exercise programme.
- if you have chosen not to obtain your doctor’s permission prior to undertaking physical activity, you are doing so at your own risk and you expressly assume and accept sole responsibility for your own health and safety.
- you are declaring yourself physically and mentally sound and suffering from no condition, injury, impairment, disease, infirmity, or other illness that would prevent your participation in training sessions.
- you will raise any questions or concerns with a member of staff before participating in your first training session.

ASSUMPTION OF RISK

I acknowledge that I am aware of and willing to assume and accept the risks associated with the types of activities offered by CrossFit DeltaFox. Therefore I hereby voluntarily agree to waive claims, release and discharge CrossFit DeltaFox from any and all liability for any loss, damage, expense or injury, including death that I may suffer or my next of kin may suffer as a result of my participation in the types of activities offered by CrossFit DeltaFox, except where any such loss, damage or injury is the direct result of negligence on their part.

I accept financial responsibility for any injury that I may cause either to myself or to any other participants due to my negligence. I further agree to indemnify and hold CrossFit DeltaFox, their employees and volunteers, harmless from liability for the injury or death of any person(s) and damage to property that may result from my negligent or intentional act or omission while participating in activities offered by CrossFit DeltaFox.
