FORTIS Gym Limited - Physical Activity Readiness Questionnaire (PAR-Q)

Regular physical activity is fun and healthy, and increasingly more people are starting to become more active every day. Being more active is very safe for most people. However, some people should check with their doctor before they start becoming much more physically active.

All information received on this form will be treated as strictly confidential.

If you answered YES to one or more questions, talk to your doctor either in person or by phone before you start becoming more physically active. Tell your doctor what questions you answered yes to on this form. You may still be able to do any activity you want as long as you start slowly and build up gradually or it may be that you need to restrict your activities to those which are safe for you. If you have answered NO to all of the questions, it is reasonably safe for you to participate in physical activity, gradually building up from your current ability level. I have read, understood and accurately completed this questionnaire. In signing, I confirm that I am voluntarily engaging in an acceptable level of exercise, and I fully acknowledge that my participation involves a risk of injury.

FORTIS Gym Limited - Teens Policy | Parental Consent Form

Confidentiality: Details on this form will be held securely and will only be shared with coaches or others who need this information in order to meet the specific needs of your child.

Parents/guardians of FORTIS Teens/Girls Club members are asked to provide and update any relevant medical information, as well as alert the FORTIS staff of any injuries in writing. This data will be shared with FORTIS coaches in case they need to contact you/administer First Aid/deal with an emergency involving your child. The hard copy of your completed form will be retained by the owner of FORTIS Gym Limited until your child ceases to be part of a FORTIS Teens/Girls Club.

In signing this:
- you accept and understand that whilst every precaution is taken to ensure that accidents do not happen, FORTIS Gym Limited cannot be held responsible for any injury sustained by my child;
- you understand and accept that should the need arise, the FORTIS staff will seek emergency treatment and/or administer emergency First Aid, and that the person with parental responsibility will be contacted as soon as possible;
