I agree to participate in one or more physical fitness program(mes)/
class(es) sponsored by Warriors RV CrossFit, which may include, but not
necessarily be limited to CrossFit training. Warriors RV CrossFit made me fully
aware that the fitness programmes/classes which Warriors RV CrossFit offers
and in which I desire to participate are of a nature and kind that are
extremely strenuous and can/ may push me to the limits of my physical
abilities.
I recognise and understand that the programs/classes are not without
varying degrees of risk which may include, but are not limited to the
following:
*Injury to the musculoskeletal and/or cardio respiratory systems which can
result in serious injury or death.
*Injury or death due to negligence on the part of myself, my training
partner, or other people around me.
*Injury or death due to improper use or failure of equipment.
*Injury or death due to a medical condition, whether known or unknown by
me.
I willingly assume full responsibility for all risks that I am exposing
myself to as a result of my participation in Warriors RV CrossFit
programs/classes and accept full responsibility for any injury or death that
may result from participation in any activity, class or physical fitness
program.
I hereby certify that I know of no medical problems that would increase my
risk of illness and injury as a result of participation in a fitness program
designed by Warriors RV CrossFit. Warriors RV CrossFit informed me that there
exists the possibility of adverse physical changes during an exercise
program and I fully understand the same. Warriors RV CrossFit informed me
that these changes could include abnormal blood pressure, fainting,
disorder of heart rhythm, stroke, and in very rare instances, heart attack or
even death, and I fully understand the same. With my full understanding of
the above information, I agree to assume all risk associated with
my participation in Warriors RV CrossFit fitness programmes/classes.

Release of Liability:
In full consideration of the above-mentioned risks and hazards and in full
consideration of the fact that I am willingly and voluntarily participating in
the activities made available by Warriors RV CrossFit, and with my full
understanding of all the above, I hereby waive, release and discharge
Warriors RV CrossFit and its agents, officers, principals and employees and
volunteers, of all liability, claims, demands, actions or rights of
action, or damages of any kind related to, arising from, or in any way
connected with, my participation in Warriors RV CrossFit fitness
programmes/classes, including those allegedly attributed to the negligent
acts or omissions of the above-mentioned parties. This agreement shall be
binding upon me, my successors, representatives, heirs, executors, assigns,
or transferees. If any portion of this agreement is held invalid, I agree that
the remainder of the agreement shall remain in full legal force and effect.
I recognise that there is risk involved in the types of activities offered by
Warriors RV CrossFit. Therefore, I accept financial responsibility for any injury
that I may cause either to myself or any other participant due to my
negligence. I further agree to indemnify and hold harmless, to the
maximum extent permitted by law, Warriors RV CrossFit their principals,
agents, employees, and volunteers from liability for the injury or death of
any person(s) and damage to property that may result from my negligent
or intentional act or omission while participating in activities offered by
Warriors RV CrossFit.
I have fully read and fully understand the foregoing assumption of risk, and
release of liability and I understand that by signing it obligates me to
indemnify the parties named for any liability for injury or death of any
person and damage to property caused by my negligent or intentional act
or omission. I understand that by signing this form I am waiving valuable
legal rights.

Photography/Video Release
Participants involved in any activities offered by Warriors RV CrossFit may be
photographed or videotaped during training. The undersigned hereby
consents to the use of these photographs and/or videos without
compensation, on the Warriors RV CrossFit website or in any editorial,
promotional or advertising material produced and/or published by
Warriors RV CrossFit.
In the unlikely event of an accident, or loss or damage to any personal
effects, I acknowledge that the Company will not be liable for any direct or
indirect loss, damage or injury arising from or in connection with the
Activities and I waive all and any claims against the Company in this
respect.
I acknowledge and accept that CCTV images of all entrants will be
monitored and recorded throughout the premises for the purposes of
crime prevention and public safety.
I acknowledge that I (and the child/ren in my care if applicable) have been
provided with safety rules and advice of the Company in relation to the
Activities and that I have read and fully understand the above and accept
the terms of participation freely and voluntarily without any inducement
prior to signing/submitting this form.

Please be aware you will be responsible for the cleanliness of your
designated area and kit used - this includes any part of the rig, rings,
barbells or floor used.
Please stick to the current Government guidelines, a current update of
these guidelines will be available on the Warriors RV CrossFit blog and via
email upon request.
We have standards of behaviour and integrity. bullying, racism,
discrimination will not be tolerated at Warriors RV CrossFit Be respectful of our
community and those who inhabit it.
Please respect the coaches and other members by ensuring you book in at
all times: also booking out if you’re unable to attend (with as much notice
as possible)
If you’re feeling unwell or begin to show cold/flu like symptoms, please let
us know and stay at home, for hygiene reasons please use sanitisers
provided and ensure you’re in a clean and healthy state before arriving at
Warriors RV CrossFit.
You will be required to give 30 days’ notice of membership expiry. Please
note a billing month is required or written notice should give if you wish to
cancel the membership.
Payments
Your existing payment methods will be available for Warriors RV CrossFit to take
payments in accordance with their terms.
